Is Cataract Surgery Painful?

The most common question patients ask before cataract surgery is a very human one: is it going to hurt? As an eye surgeon in Noida who has performed over 3,000 cataract procedures, my honest answer is: no, modern cataract surgery is not painful — but let me walk you through exactly what you will feel.

What we do to make it painless

Cataract surgery today is done under topical anaesthesia — numbing eye drops instilled about 30 minutes before surgery. There are no injections around the eye, no needles in the eyelid, and no general anaesthesia for a routine case.

What you will actually feel

  • A gentle pressure on the eye — like someone lightly pressing a fingertip on your closed lid
  • Bright lights and colours as the microscope shines into your eye
  • Cool fluid running down the side of your face
  • Occasional light touch on the eyelids from the surgical drape

What you will NOT feel

Sharp pain, cutting, or the sensation of an instrument inside the eye. The cornea is beautifully anaesthetised by the drops.

How long does the discomfort last after surgery?

Most patients feel a mild gritty or watery sensation for the first 24 hours — like there is a small eyelash in the eye. Paracetamol is usually enough. By day 2 most patients feel perfectly comfortable.
