Can Cataracts Come Back After Surgery?

This is one of the most common — and most misunderstood — questions after cataract surgery. The short answer: your cataract itself cannot come back, because we removed the natural cloudy lens permanently and replaced it with a clear artificial one.

But — and this is important — a certain proportion of patients develop what is called Posterior Capsular Opacification (PCO), also known as “secondary cataract”.

What is PCO?

During cataract surgery we place the new intraocular lens (IOL) inside a delicate transparent capsule that used to hold your natural lens. In some patients, the back wall of this capsule can slowly become cloudy months or years later. Because it sits right behind the IOL, it can make your vision hazy — mimicking the original cataract.

How common is PCO?

About 20–30% of patients develop some degree of PCO within 2–5 years. It has nothing to do with the surgeon or the lens type — it is a normal biological response of the residual lens cells.

How is PCO treated?

A YAG laser capsulotomy — a 5-minute, painless, in-office laser procedure. No cutting, no injection, no eye patch. You blink, we make a small opening in the cloudy capsule, and your vision clears within hours.

Signs you may have PCO

• Vision that was clear after surgery is slowly becoming hazy again
• Glare at night, similar to the original cataract
• Difficulty reading small print despite your correct glasses

Can PCO recur again after YAG laser?

No. Once the YAG laser has created the opening, the cloudiness cannot return in that area.
