What Is Presbyopia — Why Reading Becomes Hard After 40?
Somewhere in your early to mid-40s, you notice you have to hold your phone further away to read it. You blame the lighting first. Then you blame your phone. Then you realise this is happening even in daylight. Welcome to presbyopia — the near-focusing change that happens to everyone, without exception, in the fifth decade of life.
What is actually happening?
The natural lens inside your eye is elastic when young — it changes shape to focus from far to near. With age, the lens gradually loses elasticity. By 45 most people cannot focus comfortably at reading distance without help. This is not a disease and it is not preventable — it happens to every human eye.
What are your options?
1. Reading glasses
The simplest, cheapest option. Over-the-counter readers work for those with no other refractive error. Custom prescription readers are more precise.
2. Bifocals
Two prescriptions in one lens — distance on top, near at the bottom. Old-school but still very functional.
3. Progressive lenses
A smooth gradient from distance to near, no visible line. Cosmetically the best but takes 1–2 weeks to adapt.
4. Multifocal contact lenses
Some patients do very well; others struggle with lower contrast at night.
5. Multifocal or trifocal IOLs (at cataract surgery)
If you already need cataract surgery, choosing a premium multifocal or trifocal IOL can free you from glasses at almost all distances. This is our sweet spot at Nehchal Eye Centre.
6. Refractive lens exchange
For patients over 50 with high refractive error who are not candidates for LASIK, replacing the natural lens with a premium IOL (even without cataract) can be a great choice.
The important truth
Presbyopia continues to progress until about age 65 — so your reading number will keep changing. Plan for a fresh prescription every 1–2 years.
